Blue AHS

REFERRAL

The General Practitioner (GP) recommends a medical or surgical service and refers a patient to Auckland Health Services (AHS) to expedite the referrals for specialist care or diagnostic services.

ELECTRONIC REFERRAL SENT

GP sends the patient’s referral electronically to AHS.

AHS PATIENT COORDINATOR

AHS patient coordinator receives the referral and makes contact with the patient within 2 business days.

UTILISING THE AHS PROVIDER NETWORK

AHS Patient Coordinator arranges the required healthcare appointments, including appointment pre-requisites such as radiology or blood tests.

MEDICAL ITINERARY

A medical itinerary is created outlining all appointment details and provided to the patient.

REMINDER

The day prior to the appointment, the patient receives an automated text reminder of their upcoming appointment.

REPORT CONSOLIDATION

AHS ensure that all diagnostic and clinical reports are sent electronically to the GP.
